temp "empty" large icon for use as placeholder
We need to add launcher icons for bookmarks, history, download manager, image
and cookie managers, others to the customize toolbar palette. This list may or
may not include launchers for java console, js console, dom inspector, and the
customize toolbar dialog itself. I leave that up to hyatt or blake.

Added New Tab and New Window, unfortunately we're short on icons. Done here for
0.2.

Just for the record: some (hopefully) suitable icons are attached to bug 171647.
I would have attached them to this bug had I noticed it earlier :-|
