Warning! Very edge case-y!
- Go to https://permission.site
- Find and click "Protocol Handler" - an info-bar should appear
- Find and click "Pointer Lock" - an info-popup should appear
The info-popup (see screenshot as it might be the wrong name) reading "permission.site has control of your pointer. Press ESC to take back control." is hidden behind the info-bar.
As it is non-persistent and tells the user how to get out of something I would argue it should be on top.
The only other place I can remember seeing it is the fullscreen.
