This icon is used in the Bookmarks and History menus when no site favicon can be found. It's also used by the URL bar and Tab Toolbar. I think it would likely appear in the Library as well.
During their review of the global native menubar, UX flagged this icon for being updated.

This icon is used a ton. It's the fallback that we show when a site doesn't have its own favicon. Here's a short list of places you'll see it:
- URL bar
- Pinned tabs
- Library window
- Bookmarks / History menus and sidebar
- Modals
- about:logins
- about:home
- about:sessionrestore
- about:welcomeback
- Synced tabs menus and sidebar
- Search input (for engines with no favicon)
- Bookmarks toolbar
- about:performance
