One of those frustrating ones which turns out to be something really simple.
The test was crashing because it didn't set up the profile. There were warnings in the output which would have made it really obvious, but they got lost in the general noise and it ended up dying because when trying to create an inbox at "/INBOX". Obviously, the other fix would be to run the test as superuser :-)
