Opening multiple files from Windows Explorer successfully opens only some of the files

Details
When Mozilla is NOT open (and quickstart is not enabled) and when opening
multiple files in Mozilla through Windows Explorer, browser windows are opened
for each file, but the first several files fails (appears to be a variable
number each time but roughly 2/3 of the files).
Steps to reproduce the apparent bug:
1) Ensure that Mozilla has not been started.
2) Open Windows Explorer
3) Select more than one file of a type for which Mozilla is the default
application (e.g. html, gif)
4) Right-click on the selection and select "Open".
As many windows as files selected should appear, but the first few should be
blank with the old Mozilla icon and only the last few will contain the correct view.
The file type doesn't appear to matter, however, I've only tested this with
html, jpg and gif and it occurs with all these file types.
