when you have several bookmarks from one server, only the first one gets a
favicon.  if you open the bookmarks sidebar and navigate to the folder in which
the specific bookmarks are stored, the icons will reappear one-by-one as you
disclose and re-close the folder.

Reproducible: Always

Steps to Reproduce:
0. make sure browser.chrome.favicons is set to true in prefs.js
1. store some bookmarks from apple.com in your bookmarks by dragging them into a
bookmark folder in the sidebar (it doesn't matter if they are the same page or
different pages, just the same server)
2. close the sidebar
3. restart chimera
4. go to the bookmarks menu
5. look at your new bookmarks
6. open the sidebar, repeatedly click on the disclosure triangle for the folder
in which your test bookmarks are stored
7. notice the strange behavior
Actual Results:  
I would only see the first favicon when I started, the second favicon after one
cycle of opening and closing the bookmarks folder and so on...

Expected Results:  
all of my icons should show up when they are started at launch time

Confirmed using Chimera/2002100304 on 10.1.5. Reassigning to Bookmarks.

Favicons are no longer displayed in menus or the sidebar, and this issue does
not occur in the BM toolbar (I fixed it there).
