cmd-tab to Firefox does not always focus most recently used window

I don't know what triggers this state but:
Steps to reproduce
- Open two windows, focus one
- cmd+tab to another app
- cmd+tab back to Firefox
- cmd+tilde to focus the other window in Firefox
- cmd+tab to another app
- cmd+tab back to Firefox
Expected
The most recently focused window is focus when cmd+tab back to Firefox, like other apps in macOS.
Actual
If you leave Firefox focusing certain windows, focus will returns to a different window when you cmd+tab back to Firefox. I don't know what causes a window to get into this state but it seems specific to the window. For example, if I have three windows open, A, B, & C, windows A and B may exhibit the expected behavior but returning to window C will always focus window A.
I've attached a video demonstrating the behavior.
Window managers outside of macOS tend to have each window have it's own ctrl+tab group so this is likely a macOS specific bug. I'm on macOS 11.6 and 94.0a1 Nightly 2021-09-18 and I reproduced it on FF 92.0 too.

I was able to reproduce this behavior in textedit (I attached a video) so maybe this is a macOS bug.
