1733746 - Update APZ documentation to reflect Layers removal and other recent developments

Now that Layers is gone, the APZ documentation could use an update to avoid talking about Layers.

I did a pass over the docs to this effect, and also made some other improvements / updates along the way.

The patch also makes a few other updates and improvements, such as
using the "controller thread" terminology consistently.
