Steps to reproduce:

  1. Visit a Heartland Restaurant POS-based online ordering site (ex: https://otoredmond.hrpos.heartland.us)
  2. Add at least one item to the cart, creating a carryout order.
  3. Proceed through the checkout flow, selecting "New credit card" when choosing a payment option. A modal will appear for providing CC details.

Actual results:

No fields (cardholder name, card number, expiration date, CVV, ZIP code) rendered a CC-autofill dropdown, and no CC-autofill doorhanger appeared after manually providing card details.

Expected results:

CC-autofill should have prompted to autofill a payment method.

The Bugbug bot thinks this bug should belong to the 'Toolkit::Form Autofill' component, and is moving the bug to that component. Please revert this change in case you think the bot is wrong.

Still seeing this on HRPOS sites (e.g. https://ophredmond.hrpos.heartland.us/ and https://otoredmond.hrpos.heartland.us/).

Additionally, when autofilling non-cc profile data (name, address, phone, etc.), the address isn't properly filled into the CC capture modal. Addresses are put in single-line format on line 2 of the address form (normally strictly for apt numbers/etc.) - line 1 is left unfilled. ZIP is also not filled at all.
