Similar to CL:3331522, we were hitting a case where
|allow_first_tier_oof_cache_| was true when handling fragmentation,
because |has_block_fragmentation_| was false.

To fix this, make sure that |allow_first_tier_oof_cache_| is always
false when we handle fragmentation.
