Almost all of this patch is comments and assertions (especially around the Holder). One functional change - a fix for not locking the timer on a Dispatch failure: lock the timer and re-lock the monitor in the correct order to avoid deadlock, and the match the ordering used in RemoteTimer().
Sec-moderate since this only could be an issue on Dispatch failure
