When the Bookmarks Toolbar Folder contains only a nameless folder, one cannot
create bookmarks in any folders below the BTF.

Reproducible: Always

Steps to Reproduce:
1. Open the Bookmark Manager.
2. Delete all bookmarks.
3. Delete all bookmark folders except for the Bookmarks Toolbar Folder.
4. Create a single folder inside the BTF. Give it no name.
5. Create another folder, and name it, say, "foo".
6. Move this folder below (not inside) the BTF.
7. Return to browser and drive it to some page you like.
8. Bookmarks > Add to Bookmarks...
Actual Results:  
I couldn't put the bookmark in "foo". It doesn't show up on the "Create in:" list.

Expected Results:  
"foo" should have been included on the "Create in:" list.

I keep a blank folder in my BTF so as not to take up room in the toolbar, so
this really has actually been a problem for me. Work-around: Move BTF to the
bottom of the Bookmark menu.

Actually it doesn't matter where the nameless folder is in the bookmark list. If
there is a nameless folder anywhere in the list, all folders below the nameless
folder are invisible when adding a bookmark.

I think an acceptable way to fix this would be to require names for folders.

I suppose requiring bookmarks to have a name is an OK workaround. After all,
even a period or a space is enough to avoid the problem (I only discovered this
for myself when I was trying to make a bookmark folder as small as possible so
it would fit on the Bookmarks Toolbar). There are other issues I'd far rather
devs work on.
