remove some stale documentation and code from v-c-t

Details
There is some out of date documentation in v-c-t that should have been removed some time ago. The now dead version-control-tools vcssync docs and docs around ./create-environment for example. create-environment itself can probably also be removed now, it may still be used for building documentation but other pieces of code that are specific to that file can be pruned.
