Bugzilla Phab attachments should include the repo callsign

When a Phabricator patch is added as an attachment to Bugzilla, we should include the Phab repo callsign/shortname/etc as part of the attachment name.
The main motivation here is around uplift patches. Uplift patches submitted through Phabricator will typically have the same title as the original patch, which would lead to multiple attachments in Bugzilla with the same title. Having the repo the patch is associated with as part of the title would make these attachments easier to identify on the Bugzilla side.
We could special case uplift patches by checking the repo callsign associated with the revision to see if the Phab repo has the uplift project tag on it.
