I've seen it with two separate users, and it happens quite frequently in one case. One site has the cookie domain set to "stage2.example.com" and the other has it set to ".example.com". In the latter case we've only seen it happen once or twice. In the former it happens all the time. I changed the first case to ".example.com" as well, and the problem seems to have gone away. Actually, I might be able to set up a test site if you need it.

Any progress on the test site?
My apologies, I dropped this one on the floor. At this point I'm pretty sure it was a site programming error on my part. I haven't been able to duplicate the problem since I made the fix.
