Fix some linting errors in the moz-phab README.md file. We can also make the readme look a little nicer while we're here.

  • Make the header look nicer.
  • Fix code blocks to always have a specified syntax.
  • Fix line endings and spacing.
  • Add a header for "Environment Variables".
  • Fix indentation under release announcements section.
  • Don't use raw URL for Changelog.
  • Change environmental to environment.
