When creating new tabbed browser using ctrl-T, it sometimes doesn't open even
when  done 3-6x continuously.

Reporter, please close Mozilla, delete the xul.mfl file in your profile
directory, restart Mozilla, and test this problem again. Does that solve the
problem?

Approximated Location: C:\Documents and

Data\Mozilla\Profiles\*your_profile_name*\*your_profile_folder.slt*

Removing XUL.mfasl (linux) solved this problem for me.
