Similar to Bug 1917848 but for hard blocked extensions. IF we do have both hard and soft blocked add-ons, we should use this UI (for both). See also: Figma spec.

This work has been rolled into the patches landed as part of Bug 1917848 (which have been landed in Nightly 134).
