We still have problems when we shutdown before assistive technology does. They're holding onto objects, and may let go at a bad time for us. Our accessibles can still be referencing objects in libraries which have been unloaded. This creates yet another reason to cache the accessible tree, so we can use it to iterate through and shutdown any references to dom nodes and nsIAccessible's. Related, bug 193802 and bug 180415.
This was fixed a while ago.
