Steps to reproduce

1. Reach the Address form https://www.onbuy.com
2. Manually fill in all the address form fields and submit the form(click Continue).
3. Click the change address.
4. Update the some of the fields and click Save Address.

Expected result

• The address capture doorhanger should be displayed after step 2.

Actual result

• The address capture doorhanger is not displayed after step 2. However, it is displayed after updating/add new address in the "Change Address/Edit" form, but without capturing the Name field.

• N/A

Additional notes

• There is no difference by flipping the dynamic form support prefs to false.
• Not reproducible in Chrome, the address capture doorhanger is displayed correctly after step 2.
