Other related bugs are 103920, 172110, 87447, 120739 and a few others. I will be
very specific in what is desired.

Bookmark Manager:
1. The middle toolbar needs a button "New bookmark".

The functionality already exists in the file menu. Not sure why it wasn't added
in the toolbar. Many times, it is easier to simply copy paste URLs from another
source directly as bookmarks to be visited later.

Personal Tool Bar (Browser Window)
1. Right-click (context menu) should have 'New bookmark...' command.
2. Right-click (context menu) should have 'New separator...' command.

Just like the Bookmark Manager, both of these should insert the relevant item
before the object where the context-menu was opened.

It would be wiser to have a "New >" drop-down menu like:
New >
   Bookmark
   Folder
   Separator

For best implementation, the context-menu should also open while the menu is
dropped down from either the "Bookmarks" menu of the browser or a folder from
Personal Toolbar.

Steps to Reproduce:
A1. Open Bookmark Manager
A2. No button on the toolbar to add a fresh bookmark.

B1: Bring up the context menu on Personal Toolbar (Right-click or outside-click)
B2: No command to add a bookmark
B3: No command to add a separator

C1: Create a folder on the Personal Toolbar called "BugToolBug"
C2: Create a folder within "ButToolBug" called "BugToolBug2"
C3: Return to browser to bring up context-menu on "BugToolBug"
C4: Click anywhere to cancel context-menu.
C5: Click on "BugToolBug" to display content drop-down, namely "BugToolBug2"
C6: Right-click (or outsite-click) to bring up context-menu.
C7: Context-menu does not open on drop-downs.
Actual Results:  
A: As indicated, there was no button. Had to use a roundabout method by clicking
on File, New and then "Toolbar"  to create a bookmark.

B: As indicated, there was no command. Had to open bookmark manager and perform
the necessary tasks.

C: As indicated, there was no context-menu. Had to open bookmark manager and
perform the necessary tasks.

Expected Results:  
A: The button should be there.

B: The commands should be there.

C: The context-menu should appear.
