Meta-refreshed page becomes added again to Go menu upon each refresh

* STEPS TO REPRODUCE
0) Launch Apprunner
1) View http://www.ksc.nasa.gov/shuttle/countdown/video/video45lh.html
2) Wait 5 minutes

* RESULT
 - What happened

Each time the page is refreshed (meta refresh tag), an additional instance of the
page will appear under the Go menu.

 - What was expected

Behavior similar to 4.7; Meta-refreshes shouldn't create additional instances of
the same page under the Go menu.

* REGRESSION

 - Occurs On
        Mac OS, Win32 & Linux Apprunner (1999121308 optimized build)

 - Doesn't Occur On
        Communicator 4.71 (19991129 pre-release Mac OS build)

* CONFIGURATIONS TESTED

- [Mac] Beige Power Mac G3 (266 MHz PowerPC 750), 96 MB RAM (VM on; 1 MB of VM
used), 1024x768 (Thousands of Colors), Mac OS 8.6

- [Win32] Vectra VL (233 MHz P2), 96 MB RAM, 800x600 (True Color), NT 4.0 SP5.

- [Linux] Vectra VL (266 MHz P2), 96 MB RAM. Red Hat Linux 6.0 (GNOME).

I'm verifying this bug but I wrote bug 36767 the cover new strange behavior - refreshed pages reinserting themselves as the 
current page!
