Adding a new bookmark in a complicated bookmark tree is, well, complicated.
I've found discussion on turning the "Add Bookmarks" list into an expando-tree,
but this morning a different idea hit me: Add a new menu item ("New bookmark
here" or somesuch) to the bottom of each cascaded bookmarks menu (right before
or after where the "Open in Tabs" would go).
That is, you click on the main Bookmarks menu, mouse down to a folder name,
mouse over to another folder name, and at the bottom of the menu for that folder
(for any folder, in fact) is "New bookmark here." Select that, and the current
page is inserted there, in that sub-folder, as a new bookmark. (better yet, it
calls the regular "Add Bookmarks" dialog panel with the folder already selected).

Might be an okay extension, but I've seen enough complaints about open in tabs
which is far more useful. A sane tree in the add bookmark dialog is going to
take just as long to navigate to as this proposal. In comparison, adding this
to every folder adds useless UI which is always visble, compared to the
'as-needed' UI for the tree-view add bookmark dialog.

This extension exists as https://addons.mozilla.org/firefox/11/ , but I have
found that it can become a target for incompatibilities for other tab or
bookmark extensions; and though this simple functionality is useful and
desired, I have removed this extension from my profiles, save for testing
purposes.
(I don't like the 'always visible' UI either; and I would be tempted to
solve this by making the item only appear when the Cmd key is down - once
a pseudo-standard in the Mac world, for 'I want to edit/modify this menu/widget
rather then use it').
This is still a valid request, the functionality has been tested in the
extension and found popular, thus it would be quite reasonable to add it
to the core on the grounds of simplicity and compatibility.
IMHO, precisely the same issues arise in the case of tab management and
session saving. Eventually, and when enough users want the functionality,
it ought to be considered necessary to provide one satisfactory
implementation.
Implementation by extension is very, very useful in the early stages
and definitely means that people who do not need these features, and might
consider them bloat, are never exposed to them. As time goes by, it can
happen that multiple competing implementations become a bad thing rather
than a good thing in UI terms, and an increasing load of incompatibilities
leads to balkanisation.
Even so, you may consider this to be 'gold plating' rather than anything
really important.
