When filling out the form on the above url, the pull down menu for "Country" in
in two parts.  The 1st part is "Select Region" and works ok, this will determine
the list in the 2nd part of the pull-down for the country.  After selecting
"North America" (or anything else as a test) the 2nd pull-down won't change to a
country list.  This pull-down behaves this way on Mozilla 1.6 but works just
fine with IE.

Reproducible: Always
Steps to Reproduce:
1.  Go to http://www.veritools-web.com/license_request.shtml
2.  Change to a valid region in the "Select Region" pull-down menu under the
form listing for "Country".
3.  Try to select a country from the pull-down menu to it's immediate right.
4.  No Country menu is listed.

Actual Results:  
As listed above.

Expected Results:  
I expected the right-hand pull-down menu to list countries.  NOTE:  After you
select the country this right-hand pull-down should change to "Select State".
