If you have a bookmark in the bookmarks toolbar folder that is in a HTTP
protected realm (RFC 2616) requiring a login/password, and the site has
favicon.ico, FireFox attempts to update the icon cache upon startup, and thus
prompts for a login/password to the protected site. For example,
http://editors.dmoz.org:8080/editors
Reproducible: Always
Steps to Reproduce:
1.Create a HTTP protected directory and upload favicon.ico
2. Login to the site, verify that the icon is being loaded.
3. Add site to bookmarks, in the bookmarks toolbar folder, and make sure the
bookmarks toolbar is showing.
4. Access the site via the bookmark toolbar, login so the bookmark in the
toolbar now displays favicon.ico
5. Close browser, reopen.
Actual Results:
Icon placeholder for bookmark is blank, and Firefox prompts for authentication
information.
Expected Results:
Icon should either be permanently cached with the bookmark, or should use the
saved login credentials in the password manager to silently retrieve the bookmark.

this has been reported, multiple times.
All we need to do is institute local caching of favicons and we're golden, but
that has issues too, which is part of why this isn't done yet.
