I want to be able to have all cookies rejected without confirmation,
except those coming from sites on my whitelist, which shall be accepted
without confirmation.

This is already possible.
Tools -> Options -> Privacy -> Cookies -> uncheck "enable cookies", add the ones
you do want to allow in the exceptions list, IIRC.

Oh, by the way, what about documentating that feature? E.g., you could add the
following text to the Exceptions-dialog: "Cookies from sites marked as 'Allow'
will be accepted even if you did not enable cookies."
