The File Types box at Tools|Options|Downloads|File Types should
have a checkbox to enable or disable each file association, so that
the user can disable any of them without deleting it from the list. This
will allow the user to re-enable the associations easily if necessary.

Why would the average user want to disable an association temporarily? An
advanced user might want this, but in that case the change should only be
through an extension, because the current interface is here to stay unless a
radically improved version were made *and it could be defended*. I don't see a
different version being so overwhelmingly better than what currently exists. In
any case I can almost guarantee this will be WONTFIX unless you can come up with
a good reason why the average user would want to change associations even
semi-often.

WONTFIX
if you want to disable the extension temporarily, delete it, and when you want
it permanent again, the next time you run across that filetype choose "always
use" again.
For most users, this is not something that will ever be useful, and would just
add unnecessary visual clutter.
