this is somewhat related to bug 104436.  often, the way one ends up with
multiple accounts in password manager for a single site is by inadvertently
entering incorrect account info at a site and telling password manager to store
the info.  the next time when one tries to log back in with the correct info, a
second set of account info is stored by password manager.

password manager should offer the user the option of replacing the old account
info with the new.  it should only do this if exactly one set of account info
exists for the site.  if there is already two sets of account info, it should be
assumed that the user has multiple accounts at the site and wants another.
