default.xpm should be a copy of mozicon16.xpm so it would be a 16x16 icon.  This
is so the correct sized icon (instead of a reduced 48x48 icon) will be used for
windows that do not have their own icon once Bug 242050 is resolved.

As a side note, mozicon16.xpm does in fact look different than default.xpm
reduced to 16x16.
Summary: default.xpm should be 16x16 (a copy of mozicon16.xpm → default.xpm should be 16x16 (a copy of mozicon16.xpm)
Also, we do not appear to use default.xpm for anything except for the icon for
the taskbar and upper left-hand corner of the app window.
