Address book is empty; won't let me re-add. OK button does not work

When I tried to compose an email today I found that every address in my address
book was gone. I tried to re-add them, but when I click "OK" it does nothing. I
click several times and it just sits there until I have to give up and click
cancel. The cancel button works, but OK doesn't.

Reproducible: Always
Steps to Reproduce:
1. Open address book
2. Add new contact
3. Click OK

Actual Results:  
Won't add new contact.

Expected Results:  
Add new contact
