24672 - account manager page for accounts w/out identities

we need a seperate XUL file for accounts without identities... jglick will be
designing it (after some discussion in this bug) and posting it to the above URL

As I see it, here's what fields we should KEEP:
Toplevel: Account Name (maybe) - at least make this readonly
Server: Local directory and maybe some static text about the server type
Copies & folders: None

Wow, that leaves us with lots of room on this pane. We at least need a
description of what Local Folders is, so users aren't TOO confused.

I've created am-serverwithnoidentities.xul and hooked it up.

it't got the basics:  some text, the server pretty name, and the local path.

there is no spec on this yet, so leaving it open.
