Under various circumstances, spurious text/images will appear
superimposed on the toolbar buttons. Sometimes there's text
superimposed and sometimes it's graphics. It can be made to
come and go by sweeping the mouse over the buttons from different
directions. Attached is one image of text and another of graphics.
The images/text appear to be from the toolbar buttons themselves but
displayed in the wrong button, wrong place.

your driver.

bug 224279
