When creating an email account, the SMTP settings for that account select the
first defined SMTP server, not 'Always use default server'.

This causes users to have to change each account's SMTP settings individually
if... say... their ISP all of a sudden doesn't want to let them use any SMTP
servers other than their own.  Which, can be quite a pain if someone has any
more than a couple email accounts set up.

Besides, when Thunderbird only lets you define one SMTP server in the initial
profile set up process, it implies that any given user should only need one SMTP
server for any number of email accounts.  Why not initially set it to whatever
the default SMTP server is at any given time?

Reproducible: Always
Steps to Reproduce:
1. Add a new email account.
2. Check that account's SMTP server settings after creation.
Actual Results:  
SMTP server for that account is not set to 'always use default server'.  (is
always set to the first server in the list?)

Expected Results:  

SMTP server for that account should be set to 'always use default server'.
