see attached files: given html page, i check out print preview, and the top
border is missing from the 2nd to the last page.

Same results here, top border on second page isn't there. Perhaps also worth
mentioning is that the 'DVD Katalogus' text at the top left corner on the first
page looks like it's physically off of the page.

Over to table layout... This is an issue with thead handling...

I guess the border-collapse:collapse is essential for this bug
