I have had this behavior, too. It normally occurs after I have installed a bunch
(more than 3 or 4) extensions at one time. I exit FF, wait for it to disappear
from the process stack (Win2K, FF 0.10.1 20041001), then restart FF. It takes
a couple of minutes to install, which is normal, but sometimes it never finishes.
It does not matter how long I wait (go eat dinner, shower, etc), it never displays.
I kill off the process, restart it again, and it works fine. I have not attempted
to characterize it, as far as seeing if it might have been an interrupted
extension download, or if one of those "double entries" that are sometimes in
the extension list. By double entry, I mean one that has its normal "installed"
display look, and another one that appears in all lowercase like it is the one
that is initially put in the window with the "will install after restart".

This bug has had no comments for a long time. Statistically, we have found that
bug reports that have not been confirmed by a second user after three months are
highly unlikely to be the source of a fix to the code.

While your input is very important to us, our resources are limited and so we
are asking for your help in focussing our efforts. If you can still reproduce
this problem in the latest version of the product (see below for how to obtain a

(given at the top of this mail) and add a comment to that effect, giving more
reproduction information if you have it.

If it is not a problem any longer, you need take no action. If this bug is not
changed in any way in the next two weeks, it will be automatically resolved.
