When clicking on Exit in the File menu, either all but one window close (when
some windows has tabs in it) or none at all (all 1 tab windows)

Steps to Reproduce:
1.Open multiple windows
2.CLick on File->Exit
3.

Actual Results:  
Nothing or all but one window close

Expected Results:  
Firefox shuts down completely

disable your extensions and see if it works
When disabling all the extensions (which by the way takes way to much time one
by one) all windows close as expected even if tabs are opened in some windows.
The bug would come from one extension then? Should I try activating them one by
one (time consuming!)
Found it!

Its the Download Statusbar 0.9.1, when disabled the Exit function acts as
expected, when enabled, the exit button will not even close Firefox if only one
windows has been oppened.
I indicated the bug on the extension's forum, maybe the author can fix the bug.
