When the Plugin Finder Service fails to install a plugin automatically, pressing
the 'Manual' button brings up a modal dialog box which is incomplete and
non-functional, effectively hanging Firefox.


Reproducible: Always
Steps to Reproduce:
1. Go to a page with an object requiring a plugin such as the testcase.
2. Click 'Click here to download plugin'.
3. Click 'Next' to try and install the plugin (in this case 'Macromedia
Shockwave Player 10.1')
4. Click 'I agree' and 'Next' when the license agreement has loaded.
5. Window says plugin cannot be installed.  Click 'Manual' to try and install
the plugin manually.
Actual Results:  
The following dialog box appears:

  "You have chosen to open

  which is a:
  from:

  What should Firefox do with this file?
  () Open with
  () Save to Disk
  [] Do this automatically for files like this from now on."
  
The dialog box contains buttons such as a close (X) button, OK and Cancel but
these don't do anything.  The title of the dialog box is blank, and text is
missing.  It is not possible to continue using Firefox and the whole application
must be killed and restarted.

Expected Results:  
Dialog box text is complete, and pressing the buttons has an effect.

Note: this bug can only be seen if you don't already have the required plugin
installed, and it's a plugin that Firefox cannot install automatically. 
Happened on the Macromedia Shockwave Player plugin for me.
Fixed on 10/14, your build's only a week too old.
