Has Your U.S. Visa Ever Been Cancelled or Revoked?" - "Yes" answer is always selected. This problem can also be reproduced for Mozilla 1.7.2 Mozilla/5.0 (X11; U; FreeBSD i386; en-US; rv:1.7.2) Gecko/20041018, and on Windows. There is no such problem with Opera, haven't tried any other browser.
