Clicking the "install missing plugins" button occasionally just hides the
toolbar on which it appears without showing the plugin download dialog. I
/think/ this always happens the first time a url is visited (for a particular
instance of firefox), it certainly /doesn't/ happen after I refresh or reenter
the URL.

I've only tried this with the macromedia url above.

Reproducible: Sometimes
Steps to Reproduce:
1. Create a new profile / uninstall all plugins
2. Visit http://www.macromedia.com/shockwave/welcome/
3. Click the "Install Missing Plugins" button that appears

Actual Results:  
Button and toolbar disappear

Expected Results:  
plugin download wizard dialog should be launched
