image blocking option from originating server not consistent in performance

I have image blocking turned on as load images from originating server, with no
specific websites filtered, but every now and then i see third-party images/ads
being loaded on a lot of sites...sometimes if i just reload or reopen the same
page the image may or may not re-appear.
www.comics.com is a very good example...
sometimes inside yahoo mail
i have checked whether the image is from the originating server or not by pointing 
mouse on the image, the link displayed on the status bar on one instance was 
http://media.fastclick.net/w/click.here?cid=21867&mid=49494&sid=14806&m=3&c=5140&forced_click=http://ad.doubleclick.net/jump/N296.fastclick/B1471267.2;sz=120x600;ord=1300?

and this was from the www.comics.com homepage

Reproducible: Sometimes
Steps to Reproduce:
1. turn on image blocking and choose accept images from originating server only
2. open the webpage http://www.comics.com/
3. depending on the instance, the ad images on top and right hand side may or may  
not appear
4. try reloading the page, may be 2-3 times and you'll see the inconsistency
Actual Results:  
third party images or images not from the originating server were loaded on the
page even though image blocking settings were right

Expected Results:  
accepeted images only from the originating server...
1.7 has a pretty old implementation of image blocking.. is this an issue in
current trunk builds?
comics.com uses multiple servers for advertisements, so reloading can show
different results.

sites will also use iframes or other tricks to get around the
originating-server-only restriction.

I don't think there is a bug here.  there are ways to improve the image
blocking, and there are bugs filed about them.  but in the near term, just
right-click ads and use the "block server" option.
