64.12.168.21 appears to be the culprit (AOL #3)

I've removed them from the round-robin for now.
Status: UNCONFIRMED → NEW

I'm going to resolve this as 64.12.168.21 is not a valid AOL mirror anymore. 
This does beg the question of if we should be monitoring our mirrors for HTTP
with Nagios.
