I was installed a series of plug-ins and extensions. When finished I cleared
Firefox off the screen. Restarted Firefox. And the ADDRESS URL ENTRY BOX was
gone. I cannot find a way to get it back. Not in Options or otherwise.

What build versiond do you have? Do you see the url bar when you right click the
toolbar and click on customize...?
Please use Firefox Safe Mode to start up and disable the most recent extensions.
 If one of the extensions is causing this to be hidden, disabling all extensions
should allow it to be restored.
