If someone chooses to Deny an upload or just put in some info, they should be required to add a comment.

No Action is just that. Comment isn't required. however, a denial should certainly have a comment.

This bug is fixed. :-) and yes, the checkboxes are required. because obviously, through going through them, you hit something you can't check and therefore must deny on. The comment box allows you to elaborate.
