This problem can be seen at www.nature.com as no icon at all is shown where the
favicon or the white folder would normally appear.

My own website had this problem, as my icon was 32 x 32, but I have since
corrected this problem.  Reducing the same icon file from 32 x 32 to 16 x 16
caused the icon to appear within Firefox.

Steps to Reproduce:
1. Go to www.nature.com and icon will not display.

Actual Results:  
Location where favicon should be instead has a blank white spot.

Expected Results:  
Favicon should display in location bar and in bookmarks.
Something seems corrupt about the nature.com favicon--Microsoft Paint can't open it.

Bug 175787 is about large icons being cropped, fwiw.
mozilla@jbirnbaum.com: you're kidding, right? mspaint doesn't generally support
.ico.
timeless: the MS Visual Studio editor was able to open the nature.com
favicon--resaving it from there yielded an icon that mspaint would open.  The
file size did grow substantially in doing this, fwiw.  I'm not going to pretend
to know exactly what this means, but hopefully it helps someone.

The thing is that IE6 shows nature.com's favicon fine if added to favorites. 
Also, if you save the favicon.ico to desktop, the icon for the file is the
proper favicon that should be appearing in the location bar.

A variation of this bug is the following:
If the .ico library contains two icons (a 16x16 AND a 32x32 version), the
favicon is shown in the Firefox address bar, but NOT next to the bookmarks (in
menu or in navigation bar).

The reason one would like to have the two versions in the .ico file is to
support an IE feature: when dragging an URL on the desktop, windows uses the
large icon; if only the 16x16 is present in the file, the icon on the desktop is
an ugly enlargement of the 16x16 icon.

Confirmed: I make windows ".ico" icons that support multiple icon sizes for
sites I develop, and while they display fine in the address bar, they do not
appear in the bookmarks context menu. I was wondering why this occured and I
stumbled upon this bug.

I just upgraded to Build ID: 2005101212, and this bug appears to be still there.
Additionally, existing bookmarks of sites with 32x32 icons already stored in my
bookmarks (e.g. news.bbc.co.uk) gives non-resized icons in the bookmarks menu,
i.e. they're 32x32 in the menu rather than 16x16
this bug has been annoying me for a very, very long time :(

Impossible to tell what this bug was actually about, since nature.com is using a 16x16 favicon that works perfectly, but as comment 8 points out in the most difficult to notice fashion, www.repubblica.it uses a 32x32 favicon which works fine, location bar and bookmarks, even back to 1.0.x, so it wasn't about what it claims, 32x32 favicons not working at all. And it's not about the piling on with favicons that work in the location bar, but not bookmarks, which are almost certain to be bug 258867.

If you find a site with a 32x32 favicon which doesn't work, please make sure it's smaller than 16kb, and if so file a new bug and attach the image file.

per the observed behavior attachment this bug is related to 32x32 icons only displaying the upper left quadrant of the icon in the Bookmarks sidebar.  To date, all the favicons seem to work correctly in all other areas except this one: Bookmarks sidebar.
Oh, I foolishly thought that since comment 1 mentioned that cropping rather than scaling in trees, including the bookmark sidebar tree, was bug 175787, fixed in the trunk for Fx 3 some time ago, that it would be clear enough that, um, this ain't that.
