Default headers and footers are always printed. No way to turn them off or control what gets printed.

When printing, the default headers (HL:Title, HR:URL, FL:PageOfTotal and FR:Date&Time) always appear.
There is no way to alter them or turn them off. There is no preference pane to make changes. Editing
the "print.print_[header|footer][left|center|right]" preferences in prefs.js either directly or via about:
config has no effect. Creating appropriate entries in user.js has no effect.
Reproducible: Always
Steps to Reproduce:
1. Print any web page.
2. There is no step 2.
Actual Results:
The page content prints, slightly shrunken to accomodate the headers and footers.
Expected Results:
In an ideal world, we would be able to modify the contents of header and footer sub-elements. At the
very least, there should be a way of simply turning headers and footers OFF COMPLETELY.
