In a new account there are only two boxes for the entry of the telephone number. IE has the correct three boxes

In the registration form for new account.  The entry for the telephone number
has only two boxes (one for area code and one for exchange) and is missing the
third box for the number.  When I used Internet Explorer, the three boxes are
displayed and correctly accept the entire 10 digits.

Steps to Reproduce:
1. Enter the staples site www.staples.com
2. Go into the new account section
3. Try and enter a 10 digit telephone number

Actual Results:  
Only the area code and exchange are accepted.  The last foru digits are not accepted

Expected Results:  
It should have had three text boxes for the entry of area code, exchange and
number for a total of 10 numbers.

Actually, the third input seems to be there, just wrapped to the next line for
some reason.
