Looking at the approval queue today, I see that there are two entries for Silver Skin for TB. I think it would be good to show items with the same main.id next to each other. In this case, they cover different ranges of TB. It would be good to see these entries next to each other. What happens if someone uploads version 1.0 of a theme, and then 1.0.1. Will we auto-deny version 1.0 since it is superceded?
No, 1.0 is valid. the author can choose to remove it if they don't want it. Since we keep old versions anyway it makes sense to review it.

This applies even more as the queue gets long and people submit new versions before we're able to get to the older ones
