Sometimes Firefox creates two tab but asked to create a new tab. However, FF
handles them as one - if you colse one of the two tabs both are closed. I think
this also applies to windows, i.e., if I create a new window FF actually creates
two windows. This behaviour has been there several version, from 0.7-0.8 or so
up to and including 1.0.

I will add more details next time it happens - however, I don't think this has
something do to with a certain URL, rather it seems as FF gets stuck in some
weird state.
Use any extensions? TBE?
