exxceptions dialog in cookies options does not close pressing ok. it only closes
when pressing cancel.

Do you have an old profile (i.e. Firefox 0.8 or thereabouts) in use?

What is the error that shows up in the javascript console if you get this problem?

Yes, but was the profile created using Firefox 1.0 or have you been using the
same profile since around that time?

What you'll probably need to do is find any entries that have ports listed (i.e.
www.foo.com:8080) and remove them.  Permission manager stopped supporting ports
at some point in 2003.
I think this is a dup of bug 273698.  Well, really it's a dup of bug 281999, but
that was closed as a dup of 273698, so I guess that's where it should go to.  In
any case, it seems to be a dup of one of them...
