An example is livehttpheaders. With 0.9 installed, I uninstalled it. Without
restarting, I installed 0.10. However, when I looked in the extension manager,
LiveHTTPHeaders 0.10 was shown as "This item will be uninstalled...", and sure
enough, it was never installed. Basically, uninstallation, then upgrade, without
a restart will cause the upgrade to not install.
